HomeFree started when our Medical Director asked: "Why are so many patients admitted to acute care hospitals merely for ADL support?". Existing homecare options don't provide the service and medical supervision that patients need to thrive at home. Our proven Medically-Supervised Homecare program keeps high-needs, complex patients safely at home, at low cost to the family, and at a tremendous discount to the system.

Medically-supervised care

Each Custom Care Plan is created by our Medical Director in consultation with the patient's GP and specialists to ensure all medical issues are closely followed. We do not replace the Patient Medical Home, but instead are an extension to it. All MD services are 100% covered by Alberta Health according to the SOMB.

A desirable alternative to assisted-living

Essentially all patients "requiring" assisted-living and many "requiring" long-term care can be better served by HomeFree in their own home. While patients frequently receive less than 1-hour of direct care per day in many facilities, HomeFree clients receive 4+ hours of 1-to-1 daily care. This difference means we achieve better outcomes while keeping patients at home.

Collaborative & Transparent

Our Medical Team reviews every patient during our weekly Clinical Rounds, and care providers can contact the MD for at-home issues. Our medical care is comparable and in many ways exceeds the attention provided at facilities. Families and GPs are regularly updated.

Much more affordable than you'd think

We assist families in accessing public programs that cover up to 85% of care costs. The cost of our program is surprisingly low, and often lower than their assisted-living alternatives. Typical care starts around $200 per week.

Hospital Visit

Our medical team will visit the patient in-hospital and work with the family to determine if homecare is appropriate for them.

Custom Care Plan and Home Preparation

We create a custom Care Plan in collaboration with the hospital team that seamlessly connects their in-hospital issues with in-home care, including home labs, and follow-ups as needed. We perform a home assessment and provide modifications and equipment as required.

Transport and Care Initiation

Unlike public services, our program can be set up with caregivers receiving the patient the moment they get home. Pillow-to-pillow transport can be arranged. Providers (HCA/LPN/RN) are matched according to clinical need, and are trained in the patient's care plan prior to starting.

Work with us!

Patients in our Discharge Program receive daily MD supervision until they stabilize. Hospital physicians can be reassured that their patients are receiving outstanding care in their home, matching or exceeding the services provided at most facilities.

91 year-old male presenting with recurrent falls

The Problem

This patient was hospitalized with rhabdomyolysis, and AKI after being found down after 6 hours. He was immobile. As it was his 3rd admission that year for falling, his hospitalist MD recommended assisted living or long-term care. His granddaughter, the primary caregiver, was burned out. The family had previously let go of public homecare due to poor service.

HomeFree's care team brought him home with 24-hour care, providing him with aggressive rehab and physiotherapy. He went from being bedbound to walking independently at home. We arranged home labs to check his creatinine. He now requires only 4-hours of daily care to maintain his strength, mostly covered by CDHCI. His out of pocket costs are under $200 weekly. His family is relieved of their caregiving burdens and they are enjoying his life and independence at home.
